Sounds like your talking about two different vents, right? Exhaust vent and 
the duct inside the trailer?
If so Suburban does make a model that should fit.
There is a model that has outlets on the side and bottom so you can adapt 
which ever you need to your existing heater duct. The exhaust vent is fixed 
in either horizontial or vertical position depending on model. So you should 
be able to find one to suit your requirements and reuse your original 
exhaust cover plate.
I have seen a unit installed in an Airstream by an SOB dealership that left 
the original vertical exhaust plate on the trailer and installed the new 
horizontal exhaust plate in an area next to it to complete their 
installation.
If you have a few RV parts places nearby go see what they have. Some only 
carry one or two models though that fit the brand trailer they sell but you 
may get lucky and they'll have one that's been sitting there for several 
years that was special ordered but never used.
