The successful method seems to be to have a welding or machine shop fab 
up a window using stock extrusions that have been secured to each other 
either by welding or riveting (I've seen both now). 

If you look at a cross section of the window frame:

It can be broken down into a 'C' channel that gives it strength and 
mates with the window opening seal, an 'L' angle that the window glass 
lays in, and another that closes against the outside of the frame 
opening. At the top is a rolled piece of sheet metal that slides into 
the retainer that acts as the hinge. 

The solution that seems the hardest to get right is what to do about 
retaining the seal that holds the glass in place.  The best solution 
was on the welded version I saw, it was a tiny 'L' angle welded in 
place to form the retainer lip.

The second hardest thing is the track that that the opener arm roller 
rides in.  The solution in all cases was to use a larger modern 
operator arm roller that just slid up and down in the side 'C' Channel.

I still think that if someone was to have the extrusion made, they 
could sell hundreds of feet of it per month, though.
