Hi Joy,
      I'm not certain that I understand your dilemma correctly but it seems that you are having a problem with 1) the oversize of the U-bolts relative to the size of your trailer frame and 2) whether the undermount couplers should be used instead of the standard couplers.  If I am correct then I think... :
      1) The way that the two types of couplers are made, they'd both use the same size U-bolt - which means you may have the wrong sized U-bolt.  I didn't need any spacers and the U-bolts fit snugly around the frame and coupler.  I too, have a 4" frame but in style "B" so I used the standard coupler.
      2) The undermount couplers allow for a corrected position of the spring bar jack and the strut.  This is because the Hensley Arrow hitch and it's parts ride closer to the ground than a conventional hitch.  Since your ball coupler is mounted low (style C), the Hensley Arrow hardware must be elevated and I believe this is what the undermount couplers accomplish.
      Your mention of a 4" standard coupler height not needing spacers may not take into account that the 4" part of the coupler follows the side of the frame and therefore would not affect space of the U-bolt.
      I think that a proper fit is essential to correct and safe operation of the hitch, especially where the couplers are concerned.  They will take a lot of load because both the springs bars and the struts are mounted to them.
