That 36xx6604 number is the Mallory part number. Not a serial number.
$125 CDN is an outrageous price for a $5 US part. Go to a motor repair
shop with the old one in hand and ask for a "motor run" capacitor to
replace this one. It might not look the same, the only critical numbers
are 4 UF and 660 volts. All the rest is insignificant information to the
function. The physical size being similar makes mounting handy.

Its unlikely the univolt will work properly without that capacitor. Some
don't consider the univolt works properly with all good parts.
