Dan,

I am quite sure that the 120V ground is actually connected to the 
trailer's metal. 3 wires - hot, neutral, ground.
The ground should be connected to the metal box, and also to the ground 
lug at the receptacle. The fuse box has an internal lug for the ground 
as well.
The negative side of the 12V system is connected to the frame and shell 
as well.
