Here's an idea: 

You could attach a patch to the underside of the floor. 
It would need to be big enough that the entire perimeter 
of the patch could be screwed into existing, good wood. 
So, this would be screwing up thru the patch, and then 
into existing floor from the underside.  

Then, from above, you would have a base to build on.  
Another patch to drop down inside the hole to bring the 
new floor up to level with the rest.  Then use epoxy, with 
fillers, such as sawdust or a specifically made epoxy 
filler, to fill in the voids and seams.
