It came from a tank supplier in Dallas Texas.  I think I found it through a 
local RV dealer  here in OKC.  The tank is mounted behind the axle.  I did 
not insulate it as such, just put some R-Max around it..  I mounted the 
outlet as a standard 3 inch outlet connection at the street side behind the 
axle..  Since the 1 1/2 inch valve is hard to get to , I leave it connected 
and have a 3 inch cap with a hose connection and a garden hose valve on that 
connection, I drain it from that small valve. No problems in the last 8 
years....
